We are celebrating Second Baptist’s 74th Anniversary. We will be viewing the movie “42” at Century Summit Sierra. To join in on this celebration, you must complete and submit this form for you and any other members of your household that plan to attend.

Please keep in mind these safety measures that will be taken day of:

• Please arrive to the theater 30 minutes prior to the start of the movie. Upon arrival, you must present your ticket to verify that you have reserved your seats.
• Our health team will take your temperature along with other pre-screening procedures before service begins. If you do not pass the pre-screening, you will be asked to return home for your safety and the safety of others.
• Masks or face coverings must be worn at all times in the building.
• There will be assigned seats to ensure at least 6 feet are between individuals and families of the same household.
• After the movie, please avoid congregating inside the building for the safety of you and others.
